We obtain your mobile number via our website, app, or through alternative channels, with the intent of using it exclusively for the communication purposes described in this document. The collection of mobile numbers is aimed at facilitating direct interactions with you, enabling us to send essential alerts, updates, promotional materials, and details relevant to our offerings.

Privacy of Mobile Numbers:

We commit to not sharing your mobile number with external parties without obtaining your explicit agreement, except under these specific scenarios:

  • Compliance with legal requirements, judicial processes, or requests from authorities may necessitate the disclosure of your mobile number.
  • During organizational changes such as mergers, acquisitions, or shifts in ownership, it’s possible your mobile number may be shared with the successor organization. We will inform you in such cases and provide an opportunity to opt-out.

Upon opting in to our messaging service by providing your mobile number, you will receive messages at a frequency that varies based on the content type, including transactional updates, promotional messages, or general service notifications. We aim to keep the frequency of these communications balanced but note that it could change over time. Messaging and Data Costs:

When you receive messages from us, you may be subject to standard messaging and data fees according to your mobile carrier’s policies. These fees are determined by your carrier, and we bear no responsibility for any resulting charges. For specifics on any charges that may apply, please consult with your mobile service provider.

1. Information We Collect

We may collect information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with you or your household (“Personal Information”) about you when you use our Site, and otherwise interact with us (collectively, “Services”). The information we collect falls into three categories: (a) information you provide to us; (b) information we collect through automated methods when you use our Services; and (c) information we collect from other sources (as further detailed below).

We do not perform functions or activities that involve the use or disclosure of Protected Health Information (“PHI”) as defined in the Health Information Portability and Accountability Act (“HIPAA”) and the information we collect from your use of our service and the Site is not subject to HIPAA regulations.

You can visit the Site without submitting Personal Information. However, Personal Information is required to use certain Services, for example, to create an online account, purchase memberships, book appointments, contact us via email or phone (where we may retain your message’s content and our response), receive or request data from us (e.g., newsletters), and respond to communications from us (e.g., surveys and promotional offers). In such circumstances, we may be unable to provide you with our Services unless you provide your Personal Information.

Information You Provide to Us

We collect Personal Information that you provide directly to us. If you elect to provide data that personally identifies you such as your full name, gender, email address, date of birth, mailing address, and phone number, payment information or any other information related to your appointment and use of our Services, such as general health information and treatment goals and concerns. We will collect, use, and share such Personal Information pursuant to this Policy and applicable law. This may occur in the following ways.

Online Accounts. To create an online account, you must submit your full name, gender, and email address, and create a password. You also will be presented with marketing opt-in consent. This data is used to populate a profile within your account.

Order & Payment Data. When you make a purchase online, such as signing up for a membership, we use a PCI-compliant payment processor to help us facilitate payment. We have contracted with such payment processor to ensure they protect and limit the use of your Personal Information. Thrive IV does not receive your credit card, debit card or other financial information when you make a payment- such information is provided directly to our payment processor. However, Thrive IV does receive information about your purchase and your contact information (e.g., name, email address, mailing address, phone number). Thrive IV will keep a record of your purchases.

Marketing Communications; Email Opt-Out. If you opt-in to receive marketing communications from us, you may submit your name, email address, mailing address, and phone number. We use this data to send you promotional and other electronic and hardcopy communications. We may use third-parties or service providers to deliver communications to you. You may opt-out of such marketing emails by using the unsubscribe link in the email or contacting us at [email protected] with “Unsubscribe” in the subject line. To opt-out of other communications (e.g., postal marketing and telemarketing), please contact us as set forth below. Opting out of marketing communications does not opt you out of non-marketing communications, such as emails about your account or transactions.

You may also elect to receive text messages from us. When you sign up to receive SMS text messages, we will send you information about promotional offers and more. To the extent you voluntarily opt-in to receive SMS notifications sent directly to your mobile phone number, we receive and store the information you provide, including your telephone number or when you read a text message. You may opt out of receiving text messages at any time by replying “STOP”.

Live Chat Conversations. If you use the chat feature available on the Site or via your account, we will collect all text and other data that you submit via that chat feature, and we may link the chat information to your account.

Feedback. When you provide comments or feedback about our Site, the content on the Site, or our Services (“Feedback”), we will not treat that Feedback as confidential, and we may use that Feedback for any purpose in our sole discretion so long as it does not personally identify you. Feedback will be used without attribution or compensation to you.

We use Personal Information that we collect for the following purposes:

As Stated or Agreed to at the Point of Collection. We may use Personal Information for the purposes stated or agreed-to (or as is obvious) at the point of collection. We use Google Ads to serve ads across various websites. Google uses Cookies to collect data about your visits to the Site to generate targeted advertisements to you on other websites that you visit. Thrive IV may collect, use, disclose, transfer and otherwise process de-identified and aggregated information that it receives or creates for any purposes in its sole discretion, in compliance with applicable laws. Thrive IV is the sole and exclusive owner of such de-identified and aggregated information, including if Thrive IV de-identifies Personal Information so that it no longer considered Personal Information under applicable laws.

6. Children’s Information.

The Site is not directed at children under 18 years of age. We do not knowingly collect, sell, use, or share Personal Information from children under 18. If we obtain actual knowledge that any Personal Information we collect has been provided by a child under the age of 16, we will promptly delete that information. If a parent or legal guardian learns that their child provided us with Personal Information without his or her consent, please contact us and we will make commercially reasonable attempts to delete such Personal Information.

Anything you post on Social Media is public information and will not be treated confidentially.
